This amendment is to correct the quantity of each CLIN, and to include instruction to potential bidders, to include the EIN. The amended Section B - Continuation is attached.

The due by date is extended by one day, to 10/20/2023.

CLIN One: Rate for FULL WEEKENDS is $___________________; A Minimum of One (1) Full Weekend to a Maximum of 54 Full Weekends will be scheduled by the IHS;

CLIN Two: Rate for WEEK NIGHT is $___________________; A Minimum of One (1) Weeknight to a Maximum of 134 Weeknights will be scheduled by the IHS;

CLIN Three: Rate for HOLIDAY is $___________________; A Minimum of One (1) Holiday to a Maximum of 15 Holidays will be scheduled by the IHS;

75H709-FB24Q-001 is issued as a request for quotation (RFQ) for Radiologic Technician Services at the Fort Belknap Service Unit (FBSU) in Harlem, Montana. This procurement is conducted pursuant to the authority of Federal Acquisition Regulation (FAR) part 12, Commercial Items in conjunction with FAR Part 13, Simplified Acquisition Procedures; FAR 37.101, Non-Personal Services Contract; and HHSAR Subpart 326.6, Acquisitions Under the Buy Indian Act.

This solicitation is initially set-aside for Indian Small Business Economic Enterprises (ISBEE) pursuant to the Buy Indian Act (25 U.S.C. 47). If IHS does not receive at least two (2) offers that are reasonably priced offers from ISBEE’s, IHS will then evaluate quotes and consider award to other small business concerns under the following 5-tier cascading set-aside (in order of first consideration to last consideration):
